USCIS Immigration Physicals Medical Examination



Overview

Navigation Medical Center is pleased to offer immigration physical exam services. Dr. Jaime Duarte and Dr Sonia Moncayo are licensed US Civil Surgeon who has been authorized by USCIS to conduct immigration physicals for the Department of Homeland Security at 3003 Navigation Blvd. Houston TX 77003. Individuals applying for citizenship or seeking Permanent Residency, or a Green Card are required to obtain an immigration physical performed by a designated Civil Surgeon prior to approval of their application. The exam results will be recorded on Form I-693 for the applicant to return to the USCIS. The exam process generally takes 5 to 8 business days, depending on tests or vaccinations required. This examination is required to establish that the applicant is not inadmissible to the United States on public health grounds.

Civil Surgeon will conduct a thorough examination to check your health and for various types of diseases and confirm vaccinations as per USCIS guidelines. Additionally, you will be tested for Tuberculosis, Syphilis and Gonorrhea.

Pre-payment of non-refundable amount of $100 will secure your spot for the initial appointment, if you no show to the appointment of you arrive more than 15 minutes late without letting the office know, you will not be able to use the $100 for the remaining balance. You can move your initial appointment with the $100 pre-payment only once as long as you call us in advance with 24 hour.



What you bring with you to your immigration medical exam?

1. Bring In your picture ID (current driver license, state ID or your passport)
2. Visa, Mastercard or cash payment (do not accept checks)
3. Immunization records if available (Doctor will advise on getting other immunizations)
4. I-693 form. You must complete first three pages of the form including your Alien registration number. A#.
5. A list of your current medications.

Q: Does Navigation Medical Center perform medical exams for immigrants from other states?
USCIS certified Dr. Jaime Duarte and Dr Sonia Moncayo as Civil Surgeons and they can perform physical exams for immigrants for all states from his office at 3003 Navigation Blvd. Houston TX 77003.

Q: Will my insurance cover the cost of the immigration physical exam? No. Insurance companies work on a medical necessity basis and this exam is requested by USCIS outside of the medical necessity range. We ask you to pay in full at the time you check in for the exam.

Q: How much does the immigration physical cost?
The cost of the basic immigration medical exam is $450. This includes blood work (test for Tuberculosis, syphilis, and gonorrhea), physical exam, and may need follow up nurse visit/doctor visit to pick up results and certified forms.
Additional services will be charged if further testing is required based on your medical exam and basic initial assessment. (chest x-ray, blood titers for immunizations, other testing or immunizations) There will also be an additional charge if a confirmation syphilis test is needed and/or other testing as required by the US Immigration Service. All fees must be paid at your time of service.

Q: How long will it take before the results and documents are ready?
Your forms will be ready in 5 to 8 business days depending on results from laboratory testing. If the tuberculosis test is positive, a chest x-ray is required, which can add another a few days to the process. Chest x-rays are administered at an additional cost depending on facility's fee schedule. You will be required to return to our office to review your results and pick up your certified forms.

Q: Can I get my chest x-ray and blood work done somewhere else and bring the results with me to my medical exam?
We required all testing to be ordered by our physicians and using our vendors and receiving platform.

Q: Are immunizations included in the price for my immigration medical exam?
No. The cost of immunizations is extra. Depending on which immunizations you are required to have based on age, gender, and condition, will determine which must be completed before your immigration exam can be completed. Immunizations can be obtained from Navigation Medical Center at the time of your exam. We will provide you with pricing information on vaccines that we can complete for you, for an additional cost or you may go to other immunization location and bring the records of vaccination.

Q: Are other laboratory testing required and included in the price for my immigration exam?
Yes. The USCIS recently announced changes to the Tuberculosis Component of the exam and now requires the Quanti-FERON Gold TB blood test. This test is drawn here and sent to our lab vendor.

Q: Is there a waiver available for applicants who cannot afford the required vaccinations?
No. The USCIS determines which are required and there are no exceptions.

Q: Does our physician have to complete both the Vaccination Table and the Results section to properly complete the vaccination portion of the Form I-693?
Yes. USCIS requires our physician to completely fill out the vaccination table and the results section. Our physician must note in the vaccination table the complete vaccination history, date(s) of vaccinations given by us, and any waiver requests.

Q: Which vaccines are required?
Vaccine requirements are mandated by USCIS and CDC. We are here only to follow their determination about vaccines. Vaccine requirements change depending on age, and past medical history. But in general, for a healthy adult (18 to 64 years old) there are 7 vaccines needed. Tdap (tetanus), Influenza Formula 2024-2025, COVID-19 formula 2024-2025, MMR, Varicella, Polio, and Hepatitis B. In older than 65 years old, Pneumonia vaccines is required.

After the Exam
Once the immigration medical examination is completed and all immunizations and/or lab tests have been performed, we will provide you with a certified form I-693 in a sealed envelope to present to the Department of Homeland Security and USCIS. DO NOT OPEN THE SEALED ENVELOPE. Turn in the sealed envelope with your immigration application. A portion of the I-693 form will be provided to you (immunization document ONLY). Typically, you will be notified of the results of your lab work (and chest x-ray, if done) within 8 business days. If you have a health condition requiring medical follow-up, clearance will NOT be granted until follow-up is completed. You will be responsible for the cost of any additional medical examinations or treatments required to fulfill the federal health requirements of the USCIS.

The Basic fee is $450 and it includes only:
1. Physical Exam and upto 1 follow up visit.
2. QuantiFERON Gold TB test in blood.
3. Syphilis Test.
4. Gonorrhea Test.
5. Vaccination Status Verification.
6. Certified and Sealed I-693 form.
7. Copy of Immunization portion of form I-693 (page 12 only).

Additional service may be required such as specialty blood testing and vaccinations and will require additional fee.
